You can download the child health examination form and the dental examination record form below. You will need the free Adobe Acrobat Reader to open and print the forms. Download it from the Adobe Web site. The forms are also available by calling the school office at (217) 523-7670.
Certificate of Child Health Examination Form
The state of Illinois recently revised the Certificate of Child Health Examination form. A physical examination is required for entrance into preschool, Kindergarten, 6th grade, and/or when transferring from an out of state school. The exam must be dated within one year prior to the date the student enters school. It may also be used for sports physicals.

Dental Examination Record Form
The dental form is now required for students entering kindergarten. A new, updated dental form is required for students entering second grade. Sixth grade students must present a new dental form. Dental forms are required for students entering kindergarten, second grade, and sixth grade.
Current first grade students will need to bring a completed dental form to school. Current fifth grade students will need to bring a completed dental form to St. Patrick School for the school year.

WINDOWS:
